1.<\/strong><\/span><\/span> <\/strong><\/span><\/strong><\/span>El paciente visita un centro de Atenci\u00f3n Primaria de Salud donde se le realiza un diagn\u00f3stico previo a la derivaci\u00f3n a los servicios deportivos municipales.<\/span><\/p>\n

2.<\/strong><\/span><\/span> Los profesionales de salud deciden las intervenciones m\u00e1s id\u00f3neas en cada caso: consejo, intervenci\u00f3n avanzada individual y\/o grupal, prescripci\u00f3n de ejercicio f\u00edsico y recomendaci\u00f3n de activos de la comunidad.<\/p>\n

3.<\/strong><\/span><\/span> DIRAYA env\u00eda a las UAEFs la informaci\u00f3n de los pacientes derivados.<\/p>\n

4.<\/strong><\/span><\/span> El paciente acude a la UAEF a la que ha sido derivado.<\/p>\n

5.<\/strong><\/span><\/span> El gestor de la UAEF da de alta al paciente como usuario y se rellena un informe con los motivos de la derivaci\u00f3n.<\/p>\n

6.<\/strong><\/span><\/span> El gestor de proyecto asigna al paciente a un educador f\u00edsico deportivo.<\/p>\n

ACTIVITAL es parte del Plan Andaluz de Prescripci\u00f3n de Actividad y Ejercicio F\u00edsico (PAPEF) previsto para el periodo 2023-2030, cuyo objetivo principal es facilitar el desarrollo de un modelo de recomendaci\u00f3n y prescripci\u00f3n digital del ejercicio f\u00edsico para mejorar la salud, contrarrestar el sedentarismo e inactividad f\u00edsica en Andaluc\u00eda, estableciendo un marco de acci\u00f3n com\u00fan entre los sistemas de salud y deportivo.<\/p>\n
